A New Option If You Have Plateaued

In March 2026, the FDA approved a higher dose of injectable Wegovy β€” 7.2mg, triple the previous maximum of 2.4mg. It was fast-tracked under a national priority program because of how significant the clinical results were.

In the STEP UP trial of over 1,400 adults with obesity, patients on the 7.2mg dose lost an average of 20.7% of their body weight. And here is the number that really stands out: approximately one in three patients lost 25% or more. That level of weight loss was previously only seen with bariatric surgery.

Who Is This For?

Wegovy HD is not a starting dose. It is specifically for people who have already been on Wegovy 2.4mg for at least four weeks, have tolerated it well, and either plateaued or need additional weight reduction. Think of it as a next step for people who are responding to semaglutide but want to go further.

One Thing to Know About

A new side effect appeared at this dose:

Dysesthesia β€” unusual skin sensations like tingling, numbness, or burning β€” occurred in 22% of patients on the 7.2mg dose, compared to 6% at 2.4mg. It was generally mild and did not cause most people to stop treatment, but it is something to be aware of and discuss with your provider if it happens.

How to Bring It Up

If you have been on Wegovy 2.4mg and feel like your progress has stalled, you now have an FDA-approved step-up option. At your next appointment, you might say: "I've been tolerating my current dose well but I've plateaued. I'd like to discuss whether the 7.2mg dose might help me reach my goals."

Your provider will consider your overall response, side-effect history, and treatment goals. Not everyone who plateaus needs a higher dose β€” sometimes a medication switch (to tirzepatide, for example) or a lifestyle adjustment is the better move. But having the option is empowering.
